Changing accounts

I am currently on my mother's family plan. I recently got married and would like to change over to my husbands plan. What is the process for doing so? Will my mother need to be in the store with us as well? I would just like to be prepared. Also, will I need to get a new number? I do not want to do so seeing as this has been my number for a very long time.
