I plan to donate a phone to a local group for use of the free 911 feature. I have done a reset but my phone number and also my username (email@example.com email address) remains. How do I delete that information? My guess is that the phone will not be reauthorized so this information might be made available to a criminal if the phone is ever "captured".

That is very nice of you. Make sure to go under the phones settings, and you will want to do a factory reset. This will set the phone back to factory settings.
Not as long as the Phone Identity screen has:
my true cell phone number and the email address shown is my full name.
With that - I can be found in two minutes on the internet.
Factory reset does not remove that phone-related data until it is activated for a new account.
I completely understand your concern. I don't actually know if getting it truly removed is an option. But I have sent off for someone with lots of knowledge to help us get this figured out. I'll be back with you as soon as I hear something.
