Call recipient can't hear me on first call

This is not a 100% consistent problem, but it does happen about 80% of the time or so.  I'll place an outgoing call, someone will answer, and they won't be able to hear me.  Then I hang up and call back and they can hear me fine.  It's becoming something of a running gag with the people I call the most often, but it gets to be SUPER annoying when it happens to me after waiting for 20+ minutes on hold during a tech support call.  So far as I've noticed the second call always gets through.

I'm open to suggestions.

Just so we're clear.  I can hear the recipient just fine, and it doesn't seem to affect incoming calls, only outgoing.
