Ok, I think I may have located the problem then. It sounds like the phone was replaced. Do you have the original packaging, or any paperwork that would list the IMEI from the original purchase?
No, it's been years since I bought the phone which I thought was the one I had all along. This is outrageous. Since Sprint (and not a 3rd party location) sold me the phone, you should have the original one on file since you obviously attach the IMEI to the accounts.
Did you buy it in November of 2016? Did you take it in for repair in September of 2018?
Yes to both, but they did not ship it off in September, they just looked at the issue and said If I wanted to possibly fix it to send it off and I declined.
It shows it was deactivated and reactivated on the same day. 9/19/18. I'm really not sure what happened. I'll submit another ticket to see if I can get this taken care of, with notes on the information we have figured out today.

Sprint seems to think this phone isn't mine even though I've only had one phone active on the account: We've found that the phone is still attached to another account, that has a balance. We're also seeing that the phone was never returned to the store, from that account. At this time, the phone doesn't meet eligibility requirements for unlock and will be relocked again.
This is insane, if this phone was never returned, how could I have it? I never activated another phone on my account. The records show that. I am leaving sprint tomorrow, this is the last straw! I suggest everyone else do the same.
